Job Title: (Lead Case Manager)

Responsibilities
Case Managers assist with the assessment of the individual served well-being including needs in the areas of mental health, physical health, living, learning, working, and social environments. Case managers travel to wherever consumers reside, whether that is at home, boarding homes, or other community based living to determine progress of IP objectives. The primary role of the case manager is to increase psychological adjustment for the individual served by empowering the person receiving services and family members with information, education, and support in order to achieve optimal functioning level and satisfaction in services.

Essential Functions

Provides appropriate information regarding community resources

Demonstrates the ability to assess the needs of the individual served

Monitors the progress of the individual served in maintaining placement in the least restrictive environment

Identifies goals and plans (IP) for appropriate services and documents progress

Provides ongoing assessments of needs

Advocates on behalf of the individual served and the family to have access to community resources

Notes progress or lack of for all person-directed planning objectives
Preferred Qualifications​

Bachelor’s degree in a mental health discipline or health and human services field;

Valid Driver's License with proof of liability insurance

At least 23 years of age or older

Post hire, must complete LES and HHSC training requirements State of Texas
